REPORTING FORMAT AND SUBMISSION OF IBADAH CAMP REPORT FOR KCIDOs
Kindly be informed that the secretariat of Ibadah Camp for K/C/D/I/Os are requested to prepare and submit the report of your respected KCDIOs to the Training Unit of SHAS Mosque, IIUM Gombak Campus
There are four (4) forms to be completed by the secretariat of Ibadah Camp of K/C/D/I/Os :-
- FORM A : Programme Evaluation Form to be filled in by every participant.
- FORM B : Attendance at every session to be recorded by the secretariat.
- FORM C : Overall Attendance Report to be completed by the secretariat.
- FORM D : Report Analysis of Programme Evaluation by participant including comments and suggestions.
The secretariat is required to submit the completed reports of Form B, C and D two (2) weeks after the programme.
