Enclosed is the checklist and related documents of LTIF honorarium claim form for Semester 1, 2021/2022.
The checklist and necessary documents can be downloaded below:-

For those who are in campus please submit the hard copy to Credited Co-Curricular Department (CCD), STADD and make sure that your documents sort accordingly and must be signed.
Please take note that:-
1) The LTIF Teaching Claim Form softcopy is filled digitally (not to print, not handwriting and not scanned as PDF). Therefore, we need your kind cooperation to automate the claim process.
2) All PDFs files must be renamed with the format "FullName_" followed by the relevant document name.
e.g. a) ZulkifliAhmad_Appointment Letter
b) AdamJohan_Claim Form
