Medical Check-up for Centre for Foundation Studies (CFS) IIUM Gambang Campus – Local Student :

1. IIUM Health and Wellness Centre (IHWC) Gambang Campus is conducting online medical health declaration only following the current pandemic situation. Please carefully read and answer accordingly.

2. All students will be required to undergo physical medical check-up at IIUM Health & Wellness Centre (IHWC) Gambang Campus ONCE physical presence in campus is allowed. Please wait for the announcement from time to time.

3. Before attending your online Ta’aruf Week, all students MUST:

• Fill in and submit this online Medical Health Declaration google form : QR Code - Form Health Declaration Student
• Print up Medical Check-Up Form and fill it in. Please bring later once physical presence in campus is allowed : Medical Check-Up Form Malaysian/ Medical Check-Up Form International

4. All students are required to bring a complete Medical Check-up Form Malaysian 2021 with a list of regular medicines or letters if there is an existing illness ONCE you are allowed to come to campus. During the scheduled physical medical check-up with IIUM Health and Wellness Centre (IHWC) Gambang Campus, please show them to the Medical Officers.

5. Kindly read and print the Health Examination Guidelines and form(8) of IHWC which is also made available in the Online Offer Letter webpage:   http://www.iium.edu.my/tawaran/

*Chest x-ray is not compulsory unless instructed by the examining medical doctor during the medical check-up at IHWC. It will be done at our centre.

 *No need to go to General Practitioner to complete your Medical Check-Up Form. Thank you.
